An incident is not only a work accident. A dangerous situation and a near miss carry just as much information, and VITS records and investigates them the same way. A few short questions on the report decide whether the event counts as a work accident, and then open the fields the official report requires.
What you can do
- Three ways to report — from the platform, from the employee portal, or anonymously through a public form opened by scanning a QR code.
- Automatic classification — a few short questions (was someone injured, was first aid given, is their normal work restricted, was temporary incapacity assigned) decide the type of event.
- An owner and a deadline, with automatic reminders.
- Root-cause analysis — the 5 Whys or an Ishikawa fishbone, or you describe the cause in your own words.
- Explanatory statements from witnesses — the employee fills in a short form in the portal or from an emailed link.
- An action plan to prevent recurrence: tasks, owners, deadlines.
- Flag a risk assessment that needs updating in light of what happened — the flag stays visible until it is done.
- Publish the lessons learned to selected employees and notify by email the people who need to know.
- Reports — a comprehensive internal PDF for every incident and the official Estonian work-accident report.
